QMS Clinical System (QCS)
Quantitative Medical Systems, Inc. (QMS) has been a major provider of information systems and support for the dialysis industry for 30 years. We provide solutions for the dialysis field both for reimbursement and clinical information management as an integrated product, for which we supply consultation and full support.
The QCS software package was developed to work withQMS Focus as a fully integrated system. The combination of the two applications makes a powerful solution to all the dialysis center's needs.
This integrated solution offers a wide range of benefits to the dialysis provider:
- Complete charge capture. Complete revenue is realized and lost charges are eliminated because the complete clinical record is part of the integrated Oracle database and all charges are available to the billing program.
- Increased revenue per treatment. Because all treatment items are recorded in QCS,QMS Focus (the reimbursement module) can sort through these data to determine which items are reimbursable based on the specific patient-payor combination.
- Reduced staff for charge entry and verification. Because data are entered by those performing the treatments, accuracy is assured, and the need for extensive staff involvement in charge verification is reduced.
- Reduced expense of addressing CMS and network demands. Because virtually all treatment and outcome data are in the QMS database, reports and analyses requested by regulatory agencies can be easily produced.
- Better access to key outcome parameters. With the electronic interface with your laboratory, all key outcome data are in the Oracle database, reducing staff effort and allowing them to rapidly review outcome parameters.
- Reduced staff effort to integrate elements of dialysis treatment. Because all treatment, laboratory, and problem elements are in the same database, there is reduced effort to comprehensively evaluate patients and anticipate clinical problems that otherwise would not be apparent until adverse events occurred.
- Improved quality of care. With integrated information that addresses all of the commonly recognized problem areas in these complex patients, improved quality initiatives are easily supported:
- Problems -- Ongoing problems are monitored with your dialysis population. Problems are passed on to future shifts and issues for individual patients are followed until they are successfully addressed
- Blood Access -- With QCS you can evaluate blood access prevalence, longevity, professional responsibility, patency, staff canulation proficiency, and other statistics in this key area of patient morbidity.
- General quality issues -- Your staff can effectively attack quality issues with QCS and your quality improvement team by providing targeted reports to your staff that make their deliberations and action plans more effective.
These benefits derive from several special features of this powerful, state-of-the-art, integrated system. Some of them are:
Charge Capture
- Because QCS uses the same database structure asQMS Focus it assures that the all the essential data are captured into the billing system when the treatment information is entered.
- It streamlines the data collection procedures for your center insuring that all the vital information is stored in one central database. By using the open architecture ofQMS Focus it insures that your billable treatment data will be fully captured without the need of any extra data collection by the billing staff.
- QCS offers complete control over all your treatment items that the clinical staff will enter and offers the flexibility to change information when guidelines change.
Patient Setup and Scheduling
- The patient information screen allows for easy entry and full recording of all the patient's registration data. This information is automatically transferred into QMS Focus, which eases the need of extra data entry.
- QCS allows for the recording of all specific needs of the patient. This would include the assignment of patient care staff, any recent diagnosis or surgery, and the recording of relevant testing.
- The scheduling of all patients is made easy through a simple data entry screen. The scheduling piece allows for the tracking of your patients and easy viewing for scheduling openings and availability.
Access Information
- QCS offers the ability to enter and track any and all access information for each individual patient. Typical information such as the type, location, insertion date, revision events, and termination date and reason are tracked by this system.
- Along with the ability to record key access information, QCS offers the ability to track any and all events related to each access point.
- Comprehensive reporting on the access events and vascular access history allows the clinical staff to track this information for follow up and care.
Order Information and Possible Problems Picked up and Passed Along
- Patient orders are all entered through a simple entry screen into QCS. QCS also offers the ability to print out a blank order form for the physician to make the order entry even simpler.
- Any problems or complications during treatments or any access events are tracked at the time of entry.
- The orders and problems can be easily tracked with the following reports in QCS.
- Treatment Compliance
- Charge Nurse Summary
- Physicians Rounding
- Medical Problem List
DOQI Guidelines Tracking
- Because the system tracks all the relevant clinical information for the patient, DOQI guidelines are easily implemented.
- QCS offers targeted reports relative to all of the key components of DOQI for use in our quality improvement intiatives. These reports would include:
- Hemodialysis Adequacy
- Anemia
- Vascular Access Tracking
- Osteodystrophy
- Nutrition
Dialysis Treatment Sheet and Ease of Treatment Data Entry
- QCS offers the ability to pre-print your dialysis treatment logs for your scheduled patients. This log contains all patient vital information, all current orders, and ongoing problems, and facilitates entry of key treatment data.
- All the vital information from previous dialysis treatments is included on the printed sheet and in one window within QCS. All the essential information -- such as treatment length, dialysate, Na modeling, pre- and post-weight, temperature, along with the ordered medications and labs -- is recorded on this log.
- Key treatment data are easily entered into the QCS application from this record.
- In some circumstances this information can be entered into QCS electronically.
Lab Result Entry and Import
- Laboratory values are generally directly imported from your laboratory vendor into QCS. Individual values (e.g., stat labs) can be entered manually.
- QCS offers the ability to import your lab results by utilizing the health industry standard batch format of HL7. This allows for uniformity in the importing of lab results from different lab companies.
Comprehensive Reporting and Review
- Along with the aforementioned reporting capabilities for orders, problems, and implementation of DOQI guidelines, QCS offers a wide range of other reports for your clinical staff.
- The ability to customize the physician rounding report allows for a personalized report for each physician or clinical staff member. It allows reporting on specific concerns for that physician or clinician in an easy-to-review format.
- The online Kardex allows the clinical staff to view all the vital information for each patient. This can also be easily printed for review or to be placed in the patient chart as a cover sheet.
